Most wild mushrooms cause a bad stomach. A few destroy the liver, and the deadly ones follow a specific and cruel pattern: violent illness, a full day of apparent recovery, then liver failure.
What it actually does
There is no single mechanism because there is no single mushroom. Amatoxins from Amanita phalloides — the death cap — inhibit RNA polymerase II, so cells cannot make protein, and the liver dies. Muscarine overstimulates the parasympathetic system. Ibotenic acid and muscimol act on the brain. Gyromitrin causes seizures and haemolysis. Correct identification changes everything, and most cases arrive without it.
How much matters

Silibinin and N-acetylcysteine are used for suspected amatoxin poisoning. Constant monitoring of liver values.
Outlook
Good for the common gastrointestinal species. Poor for confirmed amatoxin poisoning, and the outcome depends heavily on how early treatment started.
